CLINICAL DELIVERY To provide a Clinical Pharmacy Service to a variety of Medical and Surgical Specialties & participate in consultant ward rounds To work collaboratively with healthcare professionals to enhance patient safety and ensure that Medicines Management practices are safe and effective. To review patient profiles on daily basis, identify prescriptions error that might have risks to patients and to resolve relevant issues before dispensing, to recognize prescribing or administration errors, assessing potential risk & strategies to manage this risk. Making clinical interventions in prescribing or administration practices and to ensure every intervention must be documented. Monitors patients in a specific unit/specialty service which includes collecting patient-specific data, performing relevant physical assessments, interviewing patients, and identifying patient goals of therapy. (write detail note of patient reconciliation in HMIS) To work with the multidisciplinary team and educate patients / nursing staff about prescribed medication regimes (inpatient and discharge) with the aim of improving compliance and document accordingly. To achieve medicines reconciliation processes on admission level to enhances patient safety Evaluates patient-specific pharmacotherapy by evaluating appropriateness of drug therapy, assessing effectiveness, and identifying potential interactions or adverse effects. Designs a comprehensive pharmacotherapy plan for patient-specific problems and persuasively justifies recommendations based on patient-specific pharmacologic, pharmacokinetic, pharmacodynamics, pharmacogenomics, Pharmaco economic, ethical, legal and evidence-based information or standard practice guidelines. Clinical Pharmacist is responsible to enter detail note on admission in progress note and routine notes when review patient profile. While review of patient medication record, if clinical pharmacist finds minor errors in prescriptions like Frequency, routes etc can modify. To provide drug information to Health care professional and document accordingly To participate in the training of pharmacists, pharmacy technician, pharmacy resident.
